Dr. Paul Langer and the entire staff of Platinum Endodontics are pleased you have chosen us to partner with you in making your health and well-being a priority. We appreciate the trust you and your referring dentist have placed in us. We are always excited to meet new patients. Our staff are well-trained professionals, who work hard together as a team to bring you the best quality treatment in a warm, caring setting. It is our goal at Platinum Endodontics to make a difference in our patients’ lives and for you to feel like you’re part of our Platinum Endodontics family.
Please take a moment prior to your scheduled appointment to download, print, and fill out our patient information and medical history forms. Please remember to bring them with you to your next appointment, along with any referral slips or records that you may have. Scheduling
Our office is open Monday through Friday from 8:00 am. We can always be reached after hours in case of emergency. We strive to schedule your appointment as promptly as possible. If you have pain or an emergency situation, every effort will be made to see you that day.

First Visit
At your initial appointment, we will examine you and perform a few simple tests to help us diagnose your problem. Some diagnostic images will be taken which may include 3D imaging. We will then explain the recommended treatment. The procedure can often be started on the same day if you desire, and may even be completed, depending on the circumstances of the case.
IMPORTANT – A parent or legal guardian must accompany patients under the age of 18 at the consultation and all treatment visits.
PLEASE ASSIST US BY PROVIDING THE FOLLOWING INFORMATION AT THE TIME OF YOUR 1ST VISIT
- Your written referral to come here and any other referrals or records pertaining to the tooth in question.
- List of current medications you are taking.
- Any medical conditions, e.g. heart conditions, artificial heart valves, or joints, high blood pressure, rheumatic heart disease, etc.
- Dental insurance information if applicable – if you have dental insurance, please bring your dental insurance card and plan information, including your insurance ID. This saves you time and allows us to help you process your claim faster.
- If your dentist has taken x-rays, you may request that they be forwarded to our office in advance. Only the necessary diagnostic radiographs will be taken at our facility, using the latest low-exposure imaging systems.
Medications – Continue all medications prescribed by your physician for diabetes, blood pressure, thyroid problems and any other conditions. If your physician has specifically told you to stop taking any particular medication prior to treatment, e.g. a blood thinner, please follow your doctor’s instructions carefully. If there is a question about a particular medication, please call our office prior to your appointment. Please do not take any over the counter or prescription pain medication for at least 6 hours prior to your 1st appointment with us, to aid in a more accurate diagnosis.
If you have been advised by your physician or dentist to use an antibiotic as a prophylactic treatment because of mitral valve prolapse (MVP), heart murmur, hip, knee, cardiac, or other prosthesis, or rheumatic heart disease, please make sure you are on the appropriate antibiotic on the day of your appointment. Eating Before Root Canal Therapy – Be sure to eat a meal before your treatment as you may not be eating again for several hours after your procedure. If you anticipate needing Nitrous Oxide (sweet air, laughing gas) during your treatment, it is advisable to eat a lighter meal. If you are diabetic, please be sure that your blood sugar is stable and you have eaten prior to treatment. Please bring your preferred form of glucose in the event of a low blood sugar.
